Program description

Target population:

Hennepin County residents who want to apply (or recertify) for most public assistance programs. (Outreach staff members do not handle Long Term Care applications or Group Residential Housing eligibility).

Services:

  • Application and re-certification interviews for MFIP, DWP, GA, MSA, Food Support, Health Care Programs and Emergency Programs
  • Community-specific information and referrals

How to obtain Services:

Call and schedule an appointment at 612-596-1300 (do NOT enter a case number) and hold for the customer service representative. If you do enter a case number, the call is routed to the client's team phone where no one can schedule an Outreach appointment.

What to expect:

Appointments may be 2-3 weeks out. If applicants cannot wait, walk in service is available at the Century Plaza office. Outreach appointments usually book out 1-3 weeks.

Bring verification of ID, income, assets and housing costs.

The appointments last from 45-90 minutes depending on your situation and the programs you request. By the end of the appointment, you will be told if you are not eligible or be given a written list of items to verify (provide proof) in order to determine eligibility. Cash, food and emergency applications are good for 30 days. Health Care applications are good for 45 days. When you provide the required proof for each program, the team will process the case and notify you of the determination. If the verifications are not provided by the 30th or 45th day, affected programs will be denied.
