Notice of Privacy Practices

The Notice of Privacy Practices describes why HSPHD is collecting private information about you, how the information collected from you may be used and disclosed, and how you can get access to this information.

Release of Information

If you would like access to data that the Human Services and Public Health Department may have about you, you must submit the request in writing.  Following is a form that you can use to authorize the release of the information.

If you have an case worker, please submit the Release of Information form to your worker.  

If you do not know your worker or your case is closed, please send the Release of Information form to:
