Why is Hennepin County creating a health care model?

Hennepin Health 
combines social, behavioral and medical services for safety-net patients.

Hennepin County provides health and human services to more than 200,000 residents via its cooperative network, which includes Hennepin County Medical Center, NorthPoint Health and Wellness Center, Metropolitan Health Plan, and the Human Services and Public Health Department. We plan to build upon this tradition of collaboration to create an integrated health care delivery network -- called Hennepin Health -- that blends medical, behavioral health and social services in a patient-centered care model. 

By forming an accountable care organization (ACO), Hennepin will be able to:

  • Improve health outcomes
  • Improve customer and staff satisfaction
  • Reduce average costs per customer through enhanced coordination, reduction in service duplication and a model of “right care at the right time” to reduce more expensive costs downstream

Integration across systems will allow for data sharing and will help break down barriers to better outcomes. Hennepin is pursuing collaborative opportunities at the federal level, the state level, and at the local level through partnerships for improved care.
