Once a conditional offer of employment has been extended, applicants (except for a few specific positions) receive either an electronic version via email or paper version via U.S. Mail of the Hennepin County Criminal Conviction Form. 

This form must be completed and returned within 48 hours after receipt and before start of employment.

An applicant who discloses a conviction that is determined to be related to the job duties of the position will receive notice and the conditional offer of employment will be rescinded.

This procedure is part of the Hennepin County application process and not a background check. Many departments conduct their own background checks and applicants may receive additional forms to complete as part of that process.
