How to obtain services

Our services help people with developmental disabilities (DD) live in their communities as independently as possible. These include:

  • DD screenings
  • Information about community resources and county, state, & federal programs
  • Short-term service planning to help individuals and families develop a plan of care
  • Ongoing case management
  • Assistance with applying for public benefits
  • Access to Waivers (DD, Elderly, CADI, CAC and TBI waivers), Alternative Care and MA Home Care services
  • Access to information on specialized housing and funding including assisted living options

Services for Children with Autism or Developmental Disabilities

Families with disabled children can contact Hennepin County to get information about social work, financial, and community resources that may be available to help their child. Services may be available to assist families when:

  • Children have a condition such as autism that cause the child to function in a way that is similar to a child with developmental delays, or
  • Children under the age of 6 are delayed in their development such as crawling, walking and talking, or
  • Children have an IQ (intelligence quotient) score below 70

Hennepin County social workers, nurses and financial workers can assist families in determining if they are eligible for medical assistance, grants and other supportive services to provide the best care possible for their disabled child. We partner with schools, medical providers community resources, and advocacy groups to ensure that families receive appropriate supports to address their child’s needs.

Our goal is to help families care for their children in the community and plan for continuing supports for the disabled child as they grow. A social worker may be assigned to continue to assist families throughout the child’s development and to offer guidance and additional services as needed.
