In 2022, to become a YHDP community, we developed the Hennepin County Coordinated Community Plan to Prevent and End Youth Homelessness (PDF). The plan was developed by the YHDP Core Team, which included members from the Youth Action Board (YAB), non-profit organizations, Hennepin County, and other technical assistance partners. The plan represents a continued commitment and focus to make homelessness rare, brief, and non-reoccurring for youth and young adults (YYA).
Tracking progress
In 2024, the Youth Action Board worked with Hennepin County to develop the Coordinated Community Plan – Progress Report. The Progress Report is meant to be a living document. It includes status updates for all goals, objectives, and action steps outlined in the Coordinated Community Plan. This will guide our community’s ongoing and future work to prevent and end youth homelessness.
If you work with youth experiencing housing instability or homelessness in Hennepin County:- Review the Coordinated Community Plan – Progress Report to identify goals and objectives that you or your program may have impacted or influenced.
- If you have an update for any Action Steps, submit a survey.
